Abstract
MEDRIS, the MEDical Record Input System, has been developed to capture patient history and physical examination data. It uses menus and graphics to make the data collection process simple and effective. MEDRIS takes advantage of the hypermedia facilities available on the Macintosh to provide a clinical tool that 1) is easy to learn and use and 2) guides a physician step by step in collecting data in a systematic manner. The user has complete control over the data entry process and can access any portion of the system directly. The physician can start with the Chief Complaint module and proceed to Review of Systems, Past Medical History, Past Surgical History, Family History, Social History, Travel History and Physical Examination. However, these modules can be selected in any desired order. The physical examination module collects data in a hierarchical fashion using three input options: "Normal," "Normal except," and "Not examined."
